I think this question should be directed at the Health and Safety Executive.

As a database engineer and former industrial worker, I would suggest that the results of such a study might be more nuanced than you expect. Many/ most industrial accidents have MULTIPLE causes and immediate trigger-points (some perhaps more serious than others). It is not always easy to decide which causes are most important in general. You'll probably do best if you cut your list of categories down the the smallest possible number of options.

Does anyone know whether these data already exist? Categorised or uncategorised?
